Netball Horowhenua will be tracking all umpiring duties and will pay a REBATE to your Club or Association, which will be deducted from your
2007 affiliation fees. You are required to follow the correct procedure to ensure the rebate is paid out correctly.

1. Circle your Team/Name as being correct and sign the card.
2. lf you are required to do someone else’s duty, cross out the original appointment and write your own Team/Name so this can be allocated accordingly.
3. The rebate may NOT be paid out unless this procedure is followed.
The rebate is $1 per court or .50c per person.
